  1. Cancelling Appointments

If you need to cancel an appointment, please do so at least 24 hours in advance. You may be charged a cancellation fee if you cancel an appointment without sufficient notice.

PDCL is committed to protecting the confidentiality of your patient information. We will not disclose your patient information to any third party without your prior consent, except as required by law or to provide you with our services.

You have the right to access and review your medical records. You may also request that we release your medical records to another healthcare provider.

You have the right to access and review your medical records. You can request a copy of your medical records by submitting a written request to our Medical Records Department. We will provide you with your medical records within 30 days of receiving your request.

You may also request that we release your medical records to another healthcare provider. To do so, please complete a release authorization form and provide us with the contact information of the healthcare provider to whom you would like to release your records.
